There is a $15.00 minimum charge for this service, and we must have a signed MEDICAL RECORD RELEASE FORM before releasing patient records. Please allow 3 to 5 days processing time.

For annual exams or treating chronic medical issues, it is recommended your previous medical records be available for our physicians to review. Knowing your complete medical history allows us to more readily identify your problem and develop a proper treatment plan.
